I have recived a letter from pennsylvania department of human services informing me that my food stamps will be discontinued by Oct 1.16 due to not sending my yearly checking form wich i have not recived becuase i am out of state on a visit to my son in Maryland . i am back in my home at. i will be very appreciative if you could send me the form now and i will complete it right away and send it in fast mail your help is highly appreciated , thanks .
